Transcripts, Recommendations, and Supplemental Forms

In addition to your application, we require a secondary school transcript, a secondary school counselor report, a teacher recommendation from a math or a science teacher, and a teacher recommendation from an English or a social studies teacher. On this page, you can print out all of these documents. Give them to your counselor and teachers as soon as possible! Remember to hand them over in person and thank them profusely for helping you.

Also, we have forms that can accompany supplemental recommendations or materials you wish to send in, such as a recommendation from a coach, a language teacher, or a supervisor, or CDs of music, videos of dance or drama performances, or websites with your work. You can download these forms here as well. They are not required.
